Crime Victims Compensation Program or You may e-mail claim status requests to victimsservices@dac.state.ok.us. Request should include the victim's name, date of birth, claim number (if known). All decisions made by the Crime Victims Compensation Board are subject to the Open Records Act, Title 51, OS 24A.1-24A.24. In order to protect the privacy of the victim, information available on these reports is limited, and no victim names are included. Instructions to search for claim status: Open the Claim Status Report by clicking on the link below. The report will open in a new window. To search for a particular claim, you will need one of the following: the Compensation Claim Number, the patient's birthday, or the last four digits of the social security number. Click the link below to open the report. In the "find" section, enter the identifying search information. Some victims do not provide their social security number and date of birth at the time of application; to obtain the claim number, please call Victim Services at 800/745-6098. Open the Payment Status Report by clicking on the link above. The report will open in a new window. You may search for a payment using the Compensation Claim Number, the provider's account number, or the provider's Vendor ID number as listed with the Oklahoma Office of State Finance. Click the link above to open the report. In the "find" section, enter the identifying search information. Checks are grouped by the provider's Vendor ID number.
