Click on the above link to print a license. One free print is available upon renewal and after a major license change. A processing fee will be charged to print at other times. Hint: When you print the license there is also an option to save the file as a PDF on your computer so it will be available in the future. If you have already used your one free print, you may still print a license by signing up for SBS Online Services. Select Register for OLS if you are a new user (OLS has an annual service charge of approximately $5.95 annually. For details regarding their OLS services click on What is OLS? ). You may order a duplicate license from the Oklahoma Insurance Department for a fee of ½ your standard renewal fee. An order form is located on our web page under Forms. Mail the form along with a check payable to the Oklahoma Insurance Department for the appropriate fee. |

You are required to update and maintain your address information online. Per Title 36 O.S. § 1435.8 (F); a $50.00 address violation may be imposed if the mailing address is not current. IMPORTANT NOTICE FOR NON-RESIDENT LICENSEES: A non-resident licensee will need to update their resident address information 48 hours prior to submitting an address change request for Oklahoma. The resident license will affect the address on all non-resident licenses. Also, the address information entered on a renewal application does not update the address on your license. You must use the links below to actually update the information on your license. Click on the link above to update the address and follow the online instructions.
A business entity and adjusters without a resident state license is required sign up for SBS Online Services in order to process an address change request. A licensee is required to notify the OID within 30 days of a legal name change. Per Title 36 O.S. § 1435.8 (F); a $50.00 address violation may be imposed if the OID has not been notified within the required time. IMPORTANT NOTICE FOR ALL LICENSEES: Please be advised that changing your address in your resident state will not change your address in Oklahoma and vice versa. Individual licensee: Complete the Name Change Request form, signed by the licensee. Submit the form along with (1) of the following items to support the request (item should be dated within the past 30 days).
Business licensee: Complete the Name Change Request form, signed by an owner or officer of the business. The legal business name is required to be listed on the license at all times. You may also use this form to request a DBA be added to an existing license.

An EXISTING ACTIVE Oklahoma license, with time remaining until the expiration date, may be changed from Resident ( R ) to Non-Resident (NR) or vice versa. You may have only (1) Oklahoma license. The license will be designated as a Resident if you live in Oklahoma or Non-Resident is you reside outside of Oklahoma. If you do not have an EXISTING ACTIVE Oklahoma license, return to our web page and follow the instruction to submit a NEW application. Resident to Non-Resident: Click on the appropriate link below to open the paper application. Complete the form and write CHANGE FROM RESIDENT TO NON-RESIDENT on the top of the first page of the application. Submit the completed form to the above information. Non-Resident to Resident: The OID will reciprocate with other resident states as long as the license status is changed within 90 days of a legal residence move. Note for Adjusters: Oklahoma will reciprocate with most prior resident states (except NY, CA and HI) as long as the adjuster passed the adjuster exam in their resident state prior to transferring to Oklahoma.
